St. Dennis Faith Formation

Mission Statement

We endeavor to assist parents / guardians in the awesome responsibility of leading their children to a personal relationship with God through Gospel values of Jesus Christ and witnessed through their Catholic Faith.

We attempt to do this by providing experiences in:

Reading and studying sacred scripture
Teaching basic truths of the Catholic Church
Participating in the Church’s sacramental life and prayer
Relating experiences of Gospel values through loving acts of justice and service to their families, church, community, country, and world
Reverencing all God’s creation

Policies

Absence Policy

Students are expected to attend all grade level classes. The GIFT sessions are to be attended by the parent(s) and student(s). Parents are highly encouraged to promote their children’s positive, active participation in all phases and opportunities offered. Parental support and cooperation are vital to a successful program and greatly appreciated! Parents, please notify the catechists and the Faith Formation Office when a student will be absent.

Registration Fee Policy

2011-2012 registration fee for grades K-11 is $70.00 per student (parishioners) and includes student(s) and parent(s) GIFT fees. For non-parishioners, the fee is $100.00 per student.

Registration forms need to be returned to the Faith Formation Office by September 12, 2011.

Procedures for Accidents and Illnesses during Classes

Staff will instruct youth to report all injuries/illnesses
Staff shall refer the reported or observed incident(s) to the director
In cases where the youth cannot return to a class or in emergency situations, the parents/guardians are responsible for making arrangements for medical treatment and transportation.
If an emergency is severe, the staff shall call for emergency medical care before or concurrent with notification to the parents or guardians

Cancellations for Classes

Saint Dennis Faith Formation classes will be canceled:

If Madison Metropolitan School District Schools close early or are closed for the day due to weather conditions
If weather becomes a threat during the day class would held and a winter storm warning is issued

In addition, WIBA AM (1310), WMGN (98.1 FM), and WOLX (94.9 FM) radio stations will be notified. Parents are encouraged to exercise their discretion for those "other" weather situations which inevitably happen.
